Contact our Admissions Department at (423) 697-9129 or (423) 698-0221. One of our Rehabilitation Liaisons will respond to perform a pre-admission screening and coordinate transfer to our facility.

Home Evaluation
The hospital provides no cost home screenings for individuals who might benefit from rehabilitation. To facilitate a screening, the individual, a family member or a healthcare professional should call (423) 697-9129 to request a home evaluation. A liaison will visit the individual's home, complete the screening and make appropriate recommendations.

Insurance
Rehabilitation is covered in full or part by most health insurance plans, including Medicare. The hospital has contracts with most major insurance companies and is willing to negotiate with other companies as needed.

Patient Admissions
Patients are admitted seven days a week. Referrals for admission are accepted seven days a week, including all holidays.

Assessment Clinic
HealthSouth Chattanooga offers an assessment clinic for people experiencing problems with falls, weakness, balance problems or declines in functional mobility or ability to perform activities of daily living. The clinic is provided at no cost and involves screenings performed by a rehabilitation nurse and occupational or physical therapist. The findings of the screening are used to determine if the person may benefit from inpatient or outpatient rehabilitation services. Recommendations are made to the person and their physician based on these findings. Appointment times are available during business hours and usually take between 30 minutes and one hour.
